Are you an experienced Brain Injury Case Manager with desire in service design and development?
Home Care Preferred is looking to recruit an experienced case manager to assist us with design and development of our case management service offering. The suitable candidates should have the experience and knowledge of working with clients suffering from traumatic and non-traumatic brain injuries with complex needs, solicitors, insurers and other external professionals involved. You must be an experienced HCPC/PIN registered Nurse, Occupational Therapist or Physio with valid registration.
Key Tasks, Duties & Essential skills:

  • Must have effective verbal communication and excellent report writing skills.
  • Must have a logical and structured thinking style that is comfortable operating both at a detail level and at a directional level.
  • Must have strong interpersonal skills that will allow credibility to be built quickly with clients, solicitors and other external professionals and be able to build therapeutic relationship quickly.
  • Must be able to demonstrate excellent time management skills and also co-ordinate others when working to deadlines.
  • Must have the ability to supervise / manage both internal and external staff and have excellent project management skills.
  • Must to be able to apply new learning to work situation and should be IT literate.
  • Should have strong problem solving skills and the ability to manage difficult situations.
  • Must have experience dealing with clients with Acquired Brain Injury.
  • Must be able to help clients to find resources and facilitates connection with services and rehabilitation programs.
  • Must be able to advocates on behalf of a client to obtain needed services.
  • Must maintain communication with the client, professionals involved in their rehab, family and the care team to evaluate whether the plan is effective in meeting the client’s goals.
  • Must be able to organize and attend events to promote the Home Care Preferred brand, briefing PR consultants on events and monitoring results, entering Home Care Preferred to appropriate awards.
  • Must be able to produce and deliver an effective Marketing Plan (acquired brain injury service) for the company.
  • Must keep in touch with stake holders and other relevant health care professionals for referrals.
  • Working closely with the Brain & Neurological Manager, Care Manager and the Managing Director to ensure that activities are at the heart of all marketing, and that effective internal and external publicity is achieved within the Home Care Preferred brand guidelines.
  • Must deal with incoming enquiries in an efficient and timely manner, ensuring that they are comprehensively supported and enjoy the best customer experience.
  • Must work with the Brain & Neurological Manager and Care Manager to organize assessments and plan services.
  • A full clean driving license and access to a car is essential.

Minimum Required education:

  • Bachelor’s

Required experience:

  • Must have a minimum of 4 years’ experience within your field of expertise

Salary: £28000 to £ 35000
